By now you have received the purple affidavit from your student. Everyone is responsible to complete one this year. Here are some helpful tips to make the process less inconvenient.
-Please make sure affidavits ARE NOTORIZED and have forms attached BEFORE sending them to the office. Signatures will not be notarized with out a parent present.
- We only accept these 5 items as proof to show they are zoned for our school
1. A current POWER BIll is the only utility bill excepted.
2. Lease agreement with address and signature
3. Mortgage statement
4. Tax Bill
5. Voter's registration card with the name and address shown
TWO of the above items are required with the affidavit. We are able to print the voters registration and the tax bill if you come into the school. There are 2 notaries at school that can notarize if all parties are present.
Two Party affidavits must be completed at Central Office in registration. Appointments are recommended. 770-254-1700
Two party affidavits are when a person lives with another person (friend, parent, etc.) that owns/rents the home.
Call the office with any questions. Please get in at your earliest convenience. The first class to receive all affidavits will receive something special!!!
